Copy of log book - This should distinguish between emergency and elective procedures and procedures performed should be categorised as follows:
An operative summary is required to be submitted with your applications; this should be laid out in the format of the generic breakdown at the back of the log book.
Copy of Part One Certificate or copy of result letter from Part One
Copy of certificate of completion of paediatric surgery training and/or a letter from your supervisor confirming that full paediatric surgery training has been completed.
* Approved Training relates to a period spent in a recognised programme, inspected and approved by the National Association or the European Board, as fulfilling the requirements for higher training in the specialty. It is important that candidates confirm with the Exam Board that their training has been approved.
Applicants are strongly advised to seek this confirmation before commencing their application. In the case of a National Centre training not yet inspected by the European Board, references may be requested from the supervising surgeon[s].
